For any concerns outside of posted office hours, please contact your primary care provider or go to the emergency room. Any messages left on the office VOICEMAIL will be answered the following business day.
Protected Health Information (PHI) is any part of your medical record or payment history. By law, PHI must be treated with care. By law, you are to receive a copy of the Notice of Privacy Practices. In order for others to receive your medical records, you must provide a signed release.
If an appointment is not cancelled at least 24 hours in advance you will be charged a missed appointment fee.
Prescription refills are done only at appointments. Do not ever stop your medication without first talking to a medical provider. If you do not have enough medication to last until your appointment, contact your pharmacy and have them send us a refill request. We have the right to refuse the request if an appointment is necessary.
Payment is due at the time of service. We take Health Savings Account (HSA) cards, cash, debit cards, credit cards, or ecclesiastical leader pay (if arrangement is made prior to the appointment). Some therapists take insurance.
